Group Dental Insurance – EMPLOYER PAID AND VOLUNTARY
Dental coverage is available for groups as small as one life. Plan benefits vary from a low of $750 maximum per person per year to a high of $2000 maximum per person per year.
Preventative Care — usually covered at 100% with no deductible, includes: bi-annual cleanings, fluoride treatments and x-rays.
Basic Coverage — subject to a small deductible and 20% co-payment by the insured, usually covers fillings, anesthesia, oral surgery and endodontics.
Major Services — insured pays a small deductible and 50% of the costs, the plan pays the balance. Services include crowns, dentures, bridgework and restorations.
